Role Description The Director of Social Work is a full-time, on-site role based in Vandalia, OH, responsible for overseeing social services within the community and ensuring high-quality, resident-centered support. This role leads the social work team, coordinates care planning, and collaborates with nursing, therapy, and administrative staff to address psychosocial, emotional, and behavioral health needs. The Director conducts assessments, develops and monitors individualized care plans, and manages case conferences to support residents and their families through transitions, discharge planning, and ongoing care. Daily responsibilities include supervision and mentoring of social work staff, maintaining regulatory compliance and documentation standards, and serving as a key resource for residents, families, and community partners. The role also involves continuous quality improvement, participation in interdisciplinary meetings, and contributing to programs that enhance resident satisfaction and outcomes.

Qualifications

  • Demonstrated expertise in Social Work, including assessment, counseling, and advocacy skills.
  • Strong background in Care Planning and Case Management to coordinate individualized, interdisciplinary plans of care.
  • Experience delivering Social Services in long-term care, rehabilitation, or similar healthcare settings.
  • Knowledge of Behavioral Health principles to address emotional, cognitive, and psychosocial needs.
  • Bachelor's degree in social work (LSW); current state licensure or eligibility as required in Ohio.
  • Proven leadership experience managing teams, providing supervision, and fostering a collaborative work environment.
  • Excellent communication, conflict resolution, and interpersonal skills for working with residents, families, and staff.
  • Familiarity with healthcare regulations, documentation standards, and resident rights in skilled nursing and rehabilitation communities.
  • Ability to organize multiple priorities, maintain accurate records, and use electronic health record systems effectively.
  • Commitment to person-centered care, cultural sensitivity, and ethical practice in all aspects of service delivery.
