Duties

• Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patient health by observing vital signs and recognizing changes in condition
• Administer medications accurately following medical documentation and home healthcare regulations
• Support clients with memory care needs, including dementia and Alzheimer's care, ensuring a safe environment
• Document medical records using Electronic Health Record (EHR) systems while maintaining HIPAA compliance
• Provide specialized care such as catheter management, hospice support, and end-of-life assistance
• Prepare nutritious meals, assist with feeding, and perform light housekeeping tasks like cleaning and laundry to promote a healthy living space

Requirements

• Excellent communication skills in both languages
• Prior experience in assisted living, nursing homes, or home healthcare settings is preferred
• Knowledge of medical terminology, medication administration, and vital signs monitoring
• Familiarity with EMR (Electronic Medical Records) systems and medical documentation standards
• Ability to perform physical tasks including heavy lifting, using Hoyer lifts, or assisting with transfers
• Certification in CPR, Basic Life Support (BLS), First Aid, and hospice or dementia care is highly desirable
• Experience working with geriatric patients or individuals with developmental disabilities is a plus
• Commitment to providing compassionate patient care while adhering to home health agency regulations and HIPAA guidelines
