Overview

We are seeking compassionate and dedicated Home Health Aides to provide exceptional in-home care to clients across various settings. This role involves supporting individuals with diverse needs, including seniors, those with disabilities, and patients requiring specialized care such as Alzheimer's or dementia. The ideal candidate will possess strong caregiving skills, medical knowledge, and a genuine desire to improve the quality of life for those they serve. Experience with EMR/EHR systems, medication administration, and patient care documentation is highly valued. Duties

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility assistance
  • Monitor and observe patient conditions, reporting changes or concerns promptly
  • Administer medications and manage treatments according to care plans
  • Perform vital signs checks such as blood pressure, pulse, and temperature
  • Support patients with memory care needs, including Alzheimer's and dementia care
  • Operate medical equipment such as Hoyer lifts and catheters safely and effectively
  • Maintain accurate medical documentation using EMR/EHR systems while ensuring compliance with HIPAA regulations
  • Assist with meal preparation and nutritional needs tailored to individual dietary requirements
  • Provide light housekeeping tasks including laundry and cleaning to maintain a safe environment
  • Support hospice or palliative care patients with compassion and dignity
  • Document patient progress and communicate effectively with healthcare teams regarding patient status

Experience

  • Prior experience in assisted living facilities, nursing homes, or home health environments preferred
  • Experience working with individuals with developmental disabilities or disabilities in general is advantageous
  • Knowledge of medical terminology, medication administration, and medical records management required
  • Certification in CPR and first aid is highly desirable
  • Familiarity with Alzheimer’s care, dementia care, hospice support, and long-term care practices preferred
  • Proven ability to work independently while maintaining excellent patient observation skills
  • Experience working with EMR/EHR systems for accurate documentation is a plus
  • Strong interpersonal skills for working compassionately with patients of all ages and backgrounds
