Job Summary

We are hiring for: LPN / Licensed Practical Nurse / Facility Based Crisis / Forsyth / DAY SHIFT / Full Time. Sign On Bonus: $2,500. Responsible for performing nursing functions and consumer care at a Facility Based Crisis Center, including assessment, care, and treatment of clients at the facility, while adhering to standards of care as defined by the Nurse Practice Act and RHA.

The Facility Based Crisis (FBC) Center is a community-based facility that serves as an alternative to inpatient hospitalization. The FBC operates 24/7/365 for adults aged 18+ experiencing behavioral, emotional, psychological, detox, or other acute crisis stabilization needs.

Job Responsibilities

  • Perform Nursing assessments and interventions, including medication oversight and administration as needed.
  • Report clients’ social, psychological, and physical behavior as indicated.
  • Implement and ensure compliance with standards of nursing practice to promote optimal health care delivery and meet regulatory requirements.
  • Implement policies and procedures to ensure organizational compliance with confidentiality regulations, HIPAA, national accreditation agencies, and continuous quality improvement.
  • Use state guidelines, the Quality Assurance Plan, and the Client Rights Policy to ensure clinical operations comply with state regulations and update plans and policies as necessary.
  • Supervise staff: interview, recommend hiring, train employees; plan, assign, and direct work; appraise performance; reward, motivate, discipline, and promote team performance; address complaints and resolve problems.

Qualifications

  • Active and current LPN licence in the state of employment.
  • Minimum two years’ experience as a nurse working with behavioral health clients (preferred).
  • Working knowledge of psychoactive substance abuse, dependency, and evidence‑based prevention and treatment techniques.
  • Valid driver’s licence and automotive insurance required; proof of vehicle may be required for transporting individuals.
  • Complete pre‑employment screening: criminal background check, registry checks (OIG exclusions database, Child Abuse Registry, Offenders Against Individuals with Developmental Disabilities), drug testing, education verification, and other credentialing as required.
