South Carolina Licensed Practical Nurse

To support chronic care management initiatives, the full-time South Carolina Licensed Practical Nurse will conduct outreach to patients with chronic conditions, coordinate care, and promote patient engagement through education and follow-up, all in a remote setting. Key Responsibilities Conduct monthly telephonic outreach to assigned CCM patient panel and document interactions in the electronic health record (EHR) Assess patient status, reinforce individualized care plans, and support medication adherence while identifying barriers to care Provide education on chronic disease management and encourage patient participation in their treatment plans Required Qualifications Active LPN licensure in good standing, preferably in South Carolina BLS (Basic Life Support) certification, if required by role Prior experience in care coordination, case management, or chronic disease management preferred Experience working with high-risk or chronic condition populations strongly preferred Proficiency in EHR documentation and care coordination workflows, with experience in Epic Systems preferred
