Job Summary:
We are looking for a Hospice Intake Coordinator in the Freehold, NJ area to join our Ennoble Care team! This position will operate on a full-time hybrid schedule. The Hospice Intake Coordinator ensures seamless hospice operations through the scheduling of hospice aides for consistent patient care and the maintenance of accurate, compliant medical records. This role directly supports clinical teams, contributes to quality of patient care, and ensures documentation and scheduling processes meet regulatory, compliance, and operational standards.
The ideal candidate is organized, proactive, and responsive, with excellent communication skills and strong attention to detail.
Responsibilities:
Manage daily and weekly schedules for hospice aides
Adjust coverage due to call-offs, new admissions, or changing patient needs
Maintain clear communication between aides, nurses, and supervisors
Organize and maintain medical records per CMS and HIPAA compliance guidelines
Audit charts for accuracy and completeness
Process incoming documentation and resolve missing or incomplete records
Support reporting, census updates, and administrative coordination
Conduct outreach to newly enrolled patients and/or their families to gather detailed clinical information, including past medical history, diagnoses, allergies, and current medication lists.
Confirm and update medication information through direct communication with patients, family members, pharmacies, or providers as needed.
Collaborate with the interdisciplinary care team (e.g., physicians, nurse practitioners, case managers) to ensure all pertinent clinical data is obtained prior to the first provider visit.
Identify and escalate any urgent clinical concerns to the appropriate team members for timely intervention.
Ensure that all required intake documentation is completed and aligned with regulatory and organizational standards.
Support coordination of care transitions by communicating relevant patient history to internal stakeholders.
Rescheduling patient appointments as needed
Review and/or update patient insurance information.
Qualifications:
Prior healthcare, hospice, or medical office experience preferred
Strong organization and time management abilities
Excellent communication skills across diverse teams
Proficient with EMR systems and scheduling tools
Knowledge of HIPAA-compliant documentation practices
Ability to work in office on a hybrid schedule
Starting pay: $50,000 per year
