With our corporate office located in Perrysburg, Ohio, ProHealth currently services Northwest Ohio, Southwest Ohio, the Greater Cleveland, Ohio area, as well as Southeast Michigan. Communicating daily with office staff to accept new patients, complete paperwork and turn in payroll

Keeping open communication with patients and home health agencies to promote a positive and caring experience

Facilitating communication with Physical Therapist Assistant to manage patient care

Completion of Evaluation, Plan of Care, Reassessment, and Discharge visits and paperwork

Current Occupational Therapist Licensure

Bachelor's degree in Occupational Therapy

Ability to travel to patient's home or facility

Home healthcare experience preferred

Ability to travel within a 40-mile radius of Toledo, OH
