Hospice Medical Social Worker (MSW) – Davie County Overview

Carolina Caring is seeking a dedicated Master of Social Work (MSW) to join our team, providing vital case management and counseling services to patients, families, and caregivers during critical moments. Responsibilities

• Perform in-depth psychosocial assessments and develop individualized care plans in collaboration with the Interdisciplinary Team, ensuring holistic and supportive care for every patient.
• Provide vital case management and counseling services to patients, families, and caregivers during critical moments.
• Focus on serving home hospice patients in Davie County.

Qualifications

• Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education (CSWE).
• At least one year of social work experience in a health care setting.

Employee Type:

Full-Time, Monday–Friday 8:30am–5pm

Travel: 75%
