Social Worker

Job Summary/Success in this role looks like:

The primary function of this position is to provide individuals, families and groups with psychosocial support needed to cope with chronic, acute, or terminal illnesses.  This includes aiding the patient to achieve an optimal level of functioning, coordination of health care services for the patient, and referrals to social and community services/programs the patient may benefit from.
