Contract | Full-Time | Day Shift | Monday–Friday, 8:00 AM – 5:00 PM | Onsite / Field-Based

Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting across an assigned territory
• Evaluate, develop, and implement individualized patient care plans in accordance with physician orders and clinical guidelines>
• Meet a weekly productivity standard of 30 points per week
• Document all patient care activities accurately and in a timely manner using HCHB (Homecare Homebase) EMR
• Participate in on-call and weekend coverage as required
• Coordinate care with interdisciplinary team members to ensure optimal patient outcomes

Required Qualifications

• Active Physical Therapist (PT) license in the state of California
• Minimum of 1 year of home health experience with a Medicare-certified facility
• Proficiency with HCHB (Homecare Homebase) EMR system
• Ability to meet weekly productivity requirements
• Availability for on-call and weekend assignments

Preferred Experience / Skills

• Familiarity with the service territory including Laguna Beach, Aliso Viejo, Dana Point, Laguna Hills, Laguna Woods, and Laguna Niguel areas
• Strong time management and self-directed work skills suited for a field-based role
