Outreach Case Manager, Palm Desert

The Case Manager supports individuals experiencing homelessness by improving their overall well-being, including emotional, physical, and financial health. This role helps clients overcome barriers such as housing instability, mental health challenges, and lack of support by connecting them to benefits, healthcare, and community resources.

This is a field-based role (80–90% travel) throughout Coachella Valley. Each day starts in Palm Desert, with travel up to 1–2 hours to meet and assist clients.
