What You'll Do:

• Provide skilled nursing visits to patients in the comfort of their own homes across Miami-Dade and Monroe County
• Perform wound care, medication management, vital signs monitoring, and patient education
• Implement and follow individualized plans of care under RN supervision
• Collaborate with our interdisciplinary team (RNs, PTs, OTs, SLPs)
• Maintain accurate and timely clinical documentation (EVV compliant)

What We're Looking For:

• Active Florida LPN license in good standing
• Home health experience preferred but we welcome new grads with the right heart for it
• Valid Florida driver's license and reliable transportation
• Strong communication and time management skills
• Bilingual (English/Spanish) a big plus in our community
• Familiarity with EVV systems a plus

Pay: $35.00 - $40.00 per hour
