Physical Therapist Assistant (PTA) – Full Time| Skilled Nursing Facility

Location: Mahoning Post Acute Nursing and Rehab

Job Type: Full Time

The Physical Therapy Assistant supports licensed Physical Therapists in delivering high‑quality rehabilitation services to residents in a skilled nursing facility. This role focuses on helping residents improve mobility, strength, functional ability, and overall quality of life through therapeutic interventions. PTAs carry out treatment plans, document progress, and collaborate with the interdisciplinary care team to ensure safe, effective, and compassionate care.

Clinical Care

Provide physical therapy treatments and therapeutic exercises as directed by the supervising Physical Therapist.

Assist with resident assessments by measuring range of motion, strength, and functional mobility when appropriate.

Implement treatment plans that may include gait training, balance activities, strengthening, and use of modalities.

Observe and report changes in resident condition to the Physical Therapist and nursing staff.

Documentation & Compliance

Accurately document treatments, resident responses, and progress in accordance with facility policies and regulatory requirements.

Resident Interaction

Provide care with patience, courtesy, and respect, even in challenging situations.

Educate residents and families on exercises, safety, and functional strategies to support independence.

Collaboration

Work closely with Physical Therapists, Occupational Therapists, Speech Therapists, nursing staff, and other team members to coordinate care.

Participate in care plan meetings and contribute to goal setting when appropriate.

Qualifications

Graduate of an accredited Physical Therapist Assistant program.

Current state PTA license in good standing.

Ability to work effectively with older adults and individuals with complex medical needs.

Physical ability to move frequently throughout the workday and assist residents safely
