Job Summary
Vanderbilt Health is hiring a Physical Therapist Assistant (PTA) to provide home health physical therapy services throughout Nashville, TN, and surrounding Middle Tennessee communities. This full time home health role offers flexible weekday hours, one‑on‑one patient care, and the opportunity to help patients improve mobility and regain independence while working with the support of a nationally recognized health system.

Responsibilities – Physical Therapist Assistant (PTA), Home Health
- Provide home health physical therapy treatments in patients’ homes
- Carry out individualized treatment plans developed by supervising Physical Therapists
- Promote mobility, functional independence, and home safety
- Educate patients and caregivers on exercises and safe mobility strategies
- Document care accurately and timely in EMR systems
- Collaborate with Physical Therapists, RNs, physicians, and other interdisciplinary care team members
- Travel throughout the assigned home health service area
Qualifications – Physical Therapist Assistant (PTA)
Required
- Active Tennessee Physical Therapist Assistant (PTA) license
- Minimum of one year of PTA experience (home health experience not required)
- Strong communication and patient‑care skills
- Ability to work independently in the home setting
- Valid driver’s license, reliable transportation, and ability to maintain a satisfactory driving record
Preferred
- Experience with EMR systems
- Home health or outpatient rehabilitation experience
- Strong understanding of functional mobility and rehabilitation techniques
