Community Case Manager - Behavioral Health Intensive Case Management

Southcentral Foundation’s Intensive Case Management is a community-based program offering support to customer-owners ages 18 and older with underlying behavioral health challenges causing difficulties in daily living. Using a strength-based approach, ICM connects customer-owners to resources and skill-building services to increase their level of independence. The goal is to assist customer-owners with developing and utilizing a sustainable community support network.
