This position offers a part-time schedule during weekday hours. We are eager to learn about your availability!

As a Hospice Social Worker (MSW), You Will:
- Act as the psychosocial support expert on the interdisciplinary care team.
- Conduct assessments and develop care plans that address the psychological, emotional, environmental, and cultural needs of patients and families.
- Provide counseling, crisis intervention, and practical resource support tailored to each patient’s individualized care plan.
- Collaborate closely with patients, families, physicians, nurses, chaplains, and other care team members to enhance the emotional and social well-being of the patient-family unit.
- Educate families on hospice services, advance care planning, grief, and coping strategies.
- Assist in long-term planning and connect families with community resources as needed.
- Continuously monitor and document psychosocial changes in the patient or family situation, recommending updates to care plans when necessary.
- Maintain ethical standards and professional judgment in end-of-life care.
About You:
Qualifications - What You'll Bring:
- Master's Degree in Social Work (MSW) from a CSWE-accredited program.
- Current state licensure as required.
- A minimum of 1 year of experience as a Social Worker in healthcare or hospice settings.
- Strong understanding of the psychosocial dynamics of illness, grief, loss, and end-of-life care.
- Ability to provide empathetic support while maintaining professional boundaries in emotionally charged situations.
- Excellent communication skills, cultural awareness, and sensitivity to diverse family systems.
Preferred Qualifications (Not Required):
- 3 to 5 years of experience as a Social Worker in hospice or healthcare.
- Experience providing support to terminally ill patients and their families.
- Advanced grief counseling or palliative care training.
