As a Home Health Licensed Practical Nurse, you will deliver exceptional patient care and perform vital medical tasks in the comfort of patients' homes.

Key Responsibilities:

  • Deliver high-quality clinical services within your scope of practice, following state laws and the established care plan, while collaborating with our dedicated healthcare team and adhering to infection control protocols.
  • Thoroughly document patient visits in the electronic medical record, ensuring compliance with regulatory and payer requirements, using our electronic visit verification system (where applicable).
  • Engage actively in care coordination by promptly reporting patient needs and changes in condition, participating in case conferences, and communicating effectively with patients, families, physicians, and other healthcare professionals.
  • Embrace diversity by working comfortably with clients regardless of age, race, creed, color, sex, disability, or financial situation.

Required Qualifications:

  • Current and unrestricted LPN licensure.
  • Valid driver's license and vehicle insurance, along with reliable transportation or access to public transit.

Preferred Qualifications:

  • Current CPR certification or the ability to obtain it within 90 days of hire.
  • Experience in home care settings.
  • Capacity to manage multiple priorities effectively.
  • Ability to work independently.
  • Strong communication, writing, and organizational skills.
  • Flexibility to work varying hours to meet client requirements.

Compensation:

Annual total cash target pay ranges from $46,564 to $69,846, with a visit point compensation structure between $22.39 and $33.58 per visit, and an hourly rate ranging from $26.86 to $40.30. This compensation is based on a full-time employment model (40 hours per week) and can vary based on individual productivity and performance.
