POSITION SUMMARY — Field Supervisor LPN

The Field Supervisor LPN is responsible for providing in-home clinical oversight and support to Personal Care Aides and Certified Nursing Assistants. This role ensures high-quality service delivery through ongoing assessments, supervision, and timely intervention for client needs. The Field Supervisor LPN also plays an integral role in staff training, documentation, and the evaluation of client care plans to promote safety, compliance, and positive health outcomes.

Work Schedule 15-25 hours per week

Monday–Friday | Work between the hours 4PM- 8PM

Saturday- Sunday | 4PM-9PM

Core Job Responsibilities

Responsibilities include, but are not limited to:

Clinical Oversight

  • Conduct clinical assessments for LGA Residents and Day Program Attendees, as needed.
  • Perform medication administration audits to ensure accuracy and compliance.
  • Complete weekly weight monitoring, recording, and trend analysis for all Day Center participants.
  • Provide clinical assessments following hospitalizations, significant changes in condition, or other critical events.
  • Review clinical data for high-acuity individuals and coordinate appropriate follow-up.
  • Manage clinical intake and admission processes.

Training & Staff Development

  • Identify training needs and deliver medical-related education to staff members.
  • Serve as a clinical resource to Personal Care Aides and CNAs, reinforcing best practices and quality standards.

SPECIFIC RESPONSIBILITIES

  • Conduct “case openings” by accompanying the assigned Homemaker/Home Health Aide (H/HHA) on all new personal care cases to provide direction and establish expectations.
  • Visit each H/HHA working with an assigned client at least every three (3) months to ensure compliance with the plan of care and evaluate the quality of services provided.
  • Update each client's Assessment and Planning form during annual site visits or whenever the client’s condition or required services change.

Job Types: Part-time, PRN

Pay: $35.00 per hour
