Full-time, 8:00 am – 5:00 pm, Monday through Friday

About the Role

Be the organizational backbone that keeps compassionate hospice care running smoothly. As a Hospice Clinical Coordinator, you'll play a vital behind-the-scenes role ensuring that clinical operations flow seamlessly - managing schedules, maintaining accurate systems, and facilitating communication that enables our field staff to focus on what matters most: patient care. This position is perfect for a detail-oriented healthcare professional who thrives on organization, excels at multitasking, and finds satisfaction in knowing that your work directly supports quality patient care. You'll be the central hub connecting clinical supervisors, field staff, and various departments to ensure nothing falls through the cracks.

What You'll Do

Coordinate Daily Clinical Operations

Schedule patients for daily visits, ensuring timely and appropriate care delivery

Maintain accurate daily census and patient tracking systems

Process all workflow daily to keep operations running smoothly

Answer and route phone calls promptly, connecting callers with appropriate staff

Monitor and address late, paused, or unaccepted visits to prevent care gaps

Manage Documentation and Systems

Accurately enter all necessary information into the EMR system

Scan and attach critical documents including 48-hour notices, DNRs, labs, and physician orders

Prepare dashboards and reports as requested by supervisors

Maintain various tracking systems to support quality care delivery

Ensure organized, efficient systems that support continuity of care

Support Clinical Leadership

Assist supervisor with Interdisciplinary Group (IDG) meeting preparation and coordination

Help coordinate staff meetings and distribute materials

Notify supervisors promptly of problems requiring administrative intervention

Assist in obtaining renewal requirements and managing compliance documentation

Identify opportunities to enhance workflow and productivity

Provide Field Staff Support

Assist field staff with supply requests and logistical needs

Provide technical support with tablets and other equipment

Perform field staff bag checks as requested

Respond quickly to field staff questions and provide guidance

Serve as a reliable resource for clinicians in the field

Maintain Communication Excellence

Respond to inquiries and provide accurate information about the organization

Ensure effective communication between clinical supervisors and other departments

Coordinate seamlessly across multiple teams and functions

Keep all stakeholders informed and aligned on operational matters

What We're Looking For

Required Qualifications

High school diploma or GED

Knowledge of medical terminology

Proficiency in Microsoft Office Suite (Outlook, Word, Excel, PowerPoint)

Ability to operate computer equipment, fax, and photocopy machines

Preferred Qualifications

Associate's degree

1 year of experience in a healthcare setting

Experience with electronic medical records (EMR) systems
