LV HOME CARE AND HOSPICE ON CALL REGISTERED NURSE (RN) - HOSPICE WEEKEND NIGHTS $10,000 SIGN ON BONUS, APPLY TODAY Summary Provides clinical support and assists in the coordination and delivery of patient care services during or after normal business hours. Performs data/order entry, clinical record reviews, and other clinical tasks related to the provision of patient care. Plans, organizes, and directs care in collaboration with the interdisciplinary team to meet the needs of the patient and family in their homes and communities. Job Duties Performs telephone triage for patients calling during or after hours utilizing critical thinking skills and sound clinical judgement Performs assessments of patient status, provides direct nursing care, and addresses immediate needs related to physical, psychosocial, and environmental factors. Provides educational opportunities for patients, families, and clinical staff focusing on end-of-life issues, palliative care, advance directives, pain management, symptom control, home care, hospice, and discharge planning. Performs data/order entry, clinical record reviews and other clinical tasks related to the provision of patient care. Provides hand-off communication to RN covering next shift, interdisciplinary team members, and/or Clinical Manager as appropriate regarding triage information, patient status updates, and patients who require a follow up visit or phone call. Minimum Qualifications Specialized Diploma NLN approved/accredited nursing program 1 year recent experience providing direct care in a healthcare setting Demonstrates knowledge and ability to care for patients based on the population served. Demonstrates good critical thinking skills and sound clinical judgement. Demonstrates analytical and problem-solving skills. Ability to perform duties with accuracy and attention to detail. Demonstrates excellent written and verbal communication skills. Demonstrates excellent customer service skills. Demonstrates team behaviors and ability to work independently. BLS - Basic Life Support AHA - American Heart Association Within 30 Days and RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire and DL - Driver's License_PA - State of Pennsylvania Upon Hire or DL - Driver's License_NJ - State of New Jersey Upon Hire Preferred Qualifications Associate’s Degree Nursing or Bachelor’s Degree Nursing 1 year home health/hospice experience and 1 year nursing experience in a clinical office setting or phone triage/on call responsibilities AMB-BC- Ambulatory Care Nursing-Board Certified - American Nurses Credentialing Center within 3 Years or CHPN - Certified Hospice and Palliative Nurse - Hospice and Palliative Nurses Association within 3 Years Physical Demands Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking.
