We're looking for an experienced Care Management Leader to oversee and develop a high-performing team of approximately 50 Care Managers and Medical Assistants supporting a large-scale Chronic Care Management (CCM) program.

Reporting directly to the Chief Operating Officer (COO), you'll lead daily operations, improve workflows, develop performance dashboards, and create a culture of accountability while ensuring exceptional patient and provider experiences.

What You'll Do

Lead High-Performing Teams

Provide leadership and oversight for approximately 50 Care Managers and Medical Assistants.

Recruit, onboard, coach, mentor, and develop team members.

Foster a culture centered on accountability, collaboration, urgency, and continuous improvement.

Conduct regular coaching sessions, performance reviews, and career development planning.

Drive Operational Excellence

Build and maintain dashboards, scorecards, and reporting tools that monitor:

Care Manager productivity

CCM minutes

Documentation quality

Regulatory compliance

Operational performance

Lead daily operational huddles focused on productivity, quality, and workload management.

Monitor team performance and proactively remove barriers impacting efficiency.

Develop scalable workflows and operational processes that improve consistency and outcomes.

Use Data to Improve Performance

Analyze productivity trends and operational metrics to identify opportunities for improvement.

Recognize high performers while coaching employees needing additional support.

Implement performance improvement plans and corrective action when appropriate.

Present operational insights and strategic recommendations to executive leadership.

Ensure Quality Compliance

Maintain compliance with CCM documentation requirements and payer guidelines.

Ensure audit readiness across all care management operations.

Balance operational productivity with outstanding patient and provider satisfaction.

Serve as an escalation point for complex patient or provider concerns.

Partner Across the Organization

Collaborate with clinical, operations, technology, and administrative teams to improve care delivery.

Evaluate and implement new technologies, workflows, and process improvements that enhance efficiency and patient outcomes.

What We're Looking For

Required Qualifications

Bachelor's degree in Healthcare Administration, Business Administration, Nursing, or a related field.

7+ years of progressive leadership experience managing high-volume, metrics-driven teams.

At least 3 years leading people managers or large operational teams.

Experience developing dashboards, KPIs, scorecards, and performance reporting.

Strong analytical skills with the ability to translate data into operational improvements.

Experience using EHR systems, care management platforms, and reporting tools.

Excellent leadership, communication, coaching, and organizational skills.

Preferred Qualifications

Master's degree (MBA, MHA, MSN, or related field).

Experience in Chronic Care Management (CCM), Population Health, Value-Based Care, or Care Management.

Clinical licensure (RN, LCSW, or similar) is a plus.

Experience leading healthcare operations, care management, patient engagement, customer service, enrollment, call center, hospitality, retail operations, or other high-volume operational environments.
