Job Summary

We are seeking a dedicated and compassionate part-time Licensed Master Social Worker (LMSW) to join our team. The ideal candidate will possess a strong background in clinical social work, with expertise in patient assessment, case management, and community resource coordination. This role offers an opportunity to make a meaningful difference in patients' lives by providing comprehensive psychosocial support within a hospice and home health setting.

Responsibilities

  • Conduct thorough patient assessments to identify psychosocial needs and develop individualized care plans.
  • Provide clinical counseling, including behavioral therapy, mental health counseling, and crisis intervention tailored to diverse patient populations.
  • Coordinate with healthcare teams to facilitate discharge planning, end-of-life care, and hospice services.
  • Support patients and families through emotional support, grief counseling, and education about community resources.
  • Maintain accurate medical documentation and records in accordance with healthcare standards.
  • Collaborate with interdisciplinary teams to ensure holistic patient care that addresses medical, psychological, and social needs.
  • Attend bi-weekly IDG Meeting on Thursday afternoon.
  • Patient visits can be scheduled to work with your schedule.

Qualifications

  • Master's degree in Social Work (MSW) from an accredited institution; licensure as an LMSW required.
  • Proven experience in clinical social work within hospital, hospice, or home health environments preferred.
  • Familiarity with medical terminology and medical documentation practices.
  • Strong interpersonal skills with the ability to work effectively with diverse populations.
  • Competency in using EMR systems for data collection and documentation; basic life support (BLS) certification required.
