Position: Certified Nursing Assistant (CNA) – Care Coordination (In-Office)

About the Role

Tru Care Management is seeking motivated and detail-oriented Certified Nursing Assistants (CNAs) to join our growing Chronic Care Management (CCM) team. This is a non-clinical, in-office position focused on coordinating care for patients with chronic conditions.

You will play a key role in improving patient outcomes by maintaining consistent communication, tracking health data, and supporting providers with documentation and care planning.

Key Responsibilities

  • Conduct monthly patient outreach via phone
  • Document all patient interactions in the EMR system
  • Track and manage patient care plans
  • Coordinate with providers, nurses, and facility staff
  • Identify changes in patient condition and elevate when appropriate
  • Ensure compliance with CMS guidelines for CCM services
  • Assist with care gap identification and follow-ups

Qualifications

  • Active CNA certification (required)
  • Strong communication and interpersonal skills
  • Comfortable working on phones for extended periods
  • Basic computer skills (EMR experience preferred)
  • Highly organized and detail-oriented
  • Ability to work in a fast‑paced, team environment
