Registered Nurse / Licensed Practical Nurse – Private Duty Home Care | 12-Hour Shifts | Sunrise, FL

Location: Sunrise, FL Position Type: Full-Time / Part-Time / Per Diem Shift: 12-hour shifts available | AM and PM shifts needed Job Description: Matrix Home Care is currently seeking experienced and compassionate Registered Nurses and Licensed Practical Nurses for private duty home care cases in Sunrise, FL. We are looking for nurses who are dependable, professional, and comfortable providing one-on-one patient care in the home setting. Both AM and PM 12-hour shifts are available. Requirements • Provide skilled nursing care in the patient’s home • Monitor patient condition and report changes as needed • Provide trach and ventilator care • Provide G-tube care and feeding support • Administer medications and treatments as ordered • Maintain accurate documentation of care provided • Follow the patient’s plan of care • Communicate with the clinical team, patient, and family as needed • Maintain a safe and supportive home care environment Requirements: • Active Florida Registered Nurse or Licensed Practical Nurse license • Ventilator and tracheostomy experience required • G-tube experience required • Prior home care or private duty nursing experience preferred • Ability to work 12-hour shifts • Reliable, compassionate, and professional • Strong communication and documentation skills Schedule: • 12-hour shifts • AM and PM shifts available • Flexible scheduling based on case needs and nurse availability Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.
