The Case Manager is responsible for the assessment of the social, emotional and environmental needs of Alabama Coordinated Health Network (ACHN) patients to improve the medical compliance and health outcomes of populations served. This role identifies barriers to medical compliance such as lack of transportation, illiteracy, or other social determinants that impact a patient's health; and ensures services are delivered and continuity of care is maintained. This position will travel to locations within the VIVA HEALTH service area through a reliable means of transportation insured in accordance with Company policy.
