We are currently seeking a dedicated Occupational Therapist (OT) to join our growing team and make a meaningful impact in the communities we serve.

Position Summary

As a Home Health Occupational Therapist, you will evaluate patients and develop individualized treatment plans designed to improve their ability to perform daily living activities safely and independently. You will work closely with patients, caregivers, physicians, and interdisciplinary team members to achieve optimal functional outcomes.

This position offers flexibility, autonomy, and the opportunity to build strong relationships with patients while providing care in their home environment.

Responsibilities

  • Perform comprehensive occupational therapy evaluations in the home setting
  • Develop individualized treatment plans based on patient needs and physician orders
  • Assess and improve patients' ability to perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
  • Provide therapeutic interventions focused on strength, coordination, balance, cognition, and functional independence
  • Recommend adaptive equipment, assistive devices, and home safety modifications
  • Educate patients, caregivers, and family members regarding treatment plans and home safety strategies
  • Monitor patient progress and modify treatment plans as appropriate
  • Complete accurate and timely clinical documentation
  • Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure continuity of care
  • Maintain compliance with agency policies and state and federal regulations

Qualifications

Required

  • Active California Occupational Therapist (OT) License
  • Graduate of an accredited Occupational Therapy program
  • Current CPR Certification
  • Valid driver's license and reliable transportation
  • Strong communication and interpersonal skills
  • Ability to work independently and manage a flexible schedule

Preferred

  • Home Health experience
  • Experience working with geriatric patients
  • Familiarity with electronic medical record (EMR) systems

Pay: $80.00 - $100.00 per hour
