Summit Home Care and Hospice is searching for an experienced and motivated Occupational Therapist (OT) to join our team. We’re committed to providing comprehensive healthcare services in the comfort of our patients’ homes to positively impact quality of life by redefining the delivery of care.

On the Road for Patient Visits & Training at the Local Office or Remotely
Responsibilities
- Evaluate patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self‑care ability, work capacity, etc.).
- Evaluate home environment for hazards or barriers to more independent living. Identify equipment needs.
- Participate in development of the total plan of care.
- For patients who plan to return to work, perform work capacity evaluation and refer to specialized vocational training if appropriate.
- Develop treatment programs and establish goals for improved function.
- Communicate the plan of care to referring physicians and other occupational therapy professionals.
- Teach new skills or retrain patients in familiar daily activities that have been lost due to illness or injury, in accordance with organization policy.
- Fabricate splints and instruct patients in the use of various types of adaptive equipment to improve function.
- Train patients in the use of prosthetic and/or orthotic devices.
- Maintain and submit appropriate clinical records, notes and reports to the physician about any changes in the patient’s condition.
- Maintain contact with other personnel involved in the patient’s care to promote coordinated, efficient care.
- Attend and contribute to in‑services, case conferences and other meetings as required by company policy to ensure coordinated and comprehensive plans of care.
- Identify patient and family/caregiver needs for other home health services and refer to them as appropriate.
- Instruct patient’s family/caregiver and other health care personnel in the treatment regime when indicated and appropriate.
- Supervise certified occupational therapy assistants according to organization policy and state regulations.
- When therapy is the only skilled service, instruct, supervise and evaluate home health aide care every two (2) weeks.
- Participate in discharge planning for patients.
Qualifications
- Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
- Registered with the National Registration Examination of the American Occupational Therapy Association.
- Must be a licensed driver with an automobile that is in good working order and insured in accordance with the organization’s requirements.
- Possess and maintain current CPR certification.
- Recent TB test, required.
- Two years of clinical experience as an occupational therapist.
- Demonstrates good verbal and written communication, and organizational skills.
