PACE Interdisciplinary Team Coordinator (Central Valley PACE - Merced)

Under the direct supervision of the PACE Director of Center Operations, the PACE Interdisciplinary Team (IDT) Coordinator will assist with and coordinate administrative and operational activities for the Central Valley PACE IDT. The PACE IDT Coordinator works closely with the PACE Center Leadership and IDT members on activities related to developing plans of care, participant enrollment, assessments, and dis-enrollment processes. Interfaces directly with IDT members, participants, and caregivers, documenting complaints/issues and assisting with resolving their immediate needs if possible.

Schedule is Monday – Friday, 8:00am – 5:00pm

Location: Central Valley PACE Center in Merced, Ca.

Compensation: $29.94 – $31.43 an hour
