Case Manager- Homeless Outreach

Salary starts at $55K, Up to 34 paid days off, and generous medical benefits. Work in partnership with a team that helps clients access needed resources. Responsible for individual, family and group services, providing high quality case management services to assist individuals to sustain recovery, access needed medical, social, legal, educational, and other services and supports. Services are delivered to an adult population who experience homelessness, SMI, substance use, co-occurring and/or medically co-morbid diagnosis. FCS utilizes the Certified Community Behavioral Health Clinic (CCBHC) model of care. As a member of a multi-disciplinary care team you will successfully support clients by providing whole person care. Bachelor's degree in social services field required.
