IRIS Consultant - Bilingual Spanish

The IRIS Consultant supports individuals in the Milwaukee area in directing their own long‑term care through the IRIS Program. This hybrid role builds trusting relationships, guides participants in developing Support and Service Plans, ensures ongoing eligibility, and connects them with community resources. Consultants maintain regular contact, document timely, travel locally, and collaborate with a supportive ICA team to help participants live independently and meaningfully.
