Licensed Practical Nurse (LPN) – Home Health

Location: Virginia Beach / Norfolk / Chesapeake/ Portsmouth/ Suffolk, VA

Schedule: Full-Time or Part-Time

Responsibilities:

• Provide skilled nursing care to patients in their homes under the supervision of an RN.
• Monitor patient health status and report changes promptly.
• Administer medications and treatments as prescribed.
• Collaborate with the care team to ensure coordinated and effective patient care.
• Educate patients and families on health maintenance and disease prevention.
• Timely completion of required documentation

Qualifications:

• **Current LPN license in the state of Virginia.**
• **Minimum 1–2 years of home health nursing experience** IMPORTANT
• Knowledge of Medicare and ACHC standards is a plus.
• Strong communication, organization, and clinical assessment skills.
• Experience with and understanding of EMR systems
• Compassionate, reliable, and dedicated to patient-centered care.
• Wound Care experience
• Lab test collection skills

Pay: $30.00 - $50.00 per hour
