Physical Therapist

About the Role

We're a Florida-licensed, ACHC-accredited skilled home health agency hiring a full-time, W-2 Physical Therapist to join our clinical team. This is a salaried position with predictable pay, benefits, and the back-office support to keep documentation manageable.

You'll work autonomously in the field with a clustered caseload, backed by an interdisciplinary team that actually communicates.

What You'll Do

• Perform skilled PT evaluations, develop individualized plans of care, and deliver treatment in patients' homes
• Set functional, measurable goals that move patients toward independence
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility
• Complete timely, accurate visit documentation in our EMR
• Coordinate with RNs, OTs, and ordering physicians as part of the care team
• Communicate status changes promptly so the team can respond

What You Bring

• Active Florida PT license in good standing
• Current BLS/CPR certification (we run an in-house AHA training site if you need to renew)
• Reliable transportation, valid FL driver's license, and auto insurance
• Strong clinical assessment, documentation, and communication skills
• Comfort working independently in the field
• Home health experience preferred — strong new grads welcome to apply

Compensation & Benefits

• Salary: $100,000 to $105,000 annually, W-2
• Productivity standard: 30 visits per week
• ETO: Paid time off accrual from day one
• Mileage reimbursement for patient travel
• Geographic clustering — we route visits to minimize windshield time

Schedule

Monday–Friday, with flexibility to manage your own visit schedule within the work week.

Pay: $100,000.00 - $105,000.00 per year
