• * Provide high-quality, patient-centered care to clients in their homes, including assistance with daily living activities, medication reminders, and personal care
• Develop and maintain positive relationships with clients, families, and healthcare teams to ensure seamless care delivery
• Collaborate with colleagues to ensure continuity of care and maintain accurate records of client services
• Participate in client care conferences and update care plans as needed
• Adhere to Bright Care Home Care's policies, procedures, and standards of care
