Location: Hollywood, FL

Schedule: Full-Time (5 days/ week)

Salary per annum: $150,000 - $200,000

Job Summary

We are seeking a dedicated and compassionate Telehealth Primary Care Nurse Practitioner/Physician Assistant to join our team in our Hollywood, FL office. This is a full-time, in-office position providing virtual primary care services to patients in long-term care and skilled nursing facilities across the country. In this role, you will deliver high-quality telehealth care to a diverse patient population while collaborating closely with interdisciplinary teams to improve patient outcomes. You will also support quality management initiatives to ensure compliance with clinical standards, best practices, and regulatory requirements.

Responsibilities

Provide high-quality telehealth primary care services to patients in long-term care and skilled nursing facilities

Conduct virtual assessments, diagnose conditions, and develop individualized treatment plans

Manage chronic diseases and acute medical conditions through telemedicine platforms

Collaborate with interdisciplinary healthcare teams to coordinate patient care

Maintain accurate and timely medical documentation in compliance with regulatory standards

Participate in quality management (QM) initiatives, including chart audits, compliance reviews, and outcome tracking

Ensure adherence to clinical protocols, company policies, and regulatory guidelines

Support performance improvement initiatives and promote best practices across care teams

Educate patients, caregivers, and facility staff regarding treatment plans and preventative care

Requirements

Active and unrestricted Nurse Practitioner (NP), Physician Assistant (PA), or Physician license

Florida license in good standing required

Ability to obtain and maintain additional state licenses as needed

Minimum of 6 months of experience in long-term care, skilled nursing, or post-acute care settings preferred

Experience with telehealth platforms and electronic medical records (EMR) preferred

Strong clinical judgment and patient engagement skills

Excellent communication and collaboration abilities

Knowledge of regulatory requirements and quality improvement processes within long-term care settings

Ability to work efficiently in a fast-paced telehealth environment
