We are seeking a female dedicated and compassionate Community Facilitator to oversee overnight supported living services for an individual with diverse needs. The ideal candidate will possess a strong background in assisted living, home and community care, and long-term care environments. This role involves providing personalized support to residents, ensuring their safety, well-being, and independence during overnight hours. The Community Facilitator will play a vital role in fostering a safe, respectful, and engaging environment that promotes residents' quality of life. Responsibilities

• Provide overnight supervision and support to resident, ensuring their safety and comfort throughout the shift.
• Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and supervision.
• Administer medications accurately following prescribed care plans while maintaining compliance with HIPAA regulations for confidentiality.
• Prepare nutritious meals and snacks aligned with individual dietary needs, including meal planning and cooking responsibilities.
• Maintain cleanliness and organization of resident living areas, adhering to sanitation standards and infection control protocols.
• Document resident interactions, health observations, and incident reports thoroughly to ensure accurate record-keeping.
• Collaborate with interdisciplinary teams—including social workers, nurses, and therapists—to develop and update personalized care plans tailored to each resident’s needs.
• Support residents’ engagement in social activities and community integration efforts to promote mental stimulation and emotional well-being.

Direct Support Professional (DSP) – Full-Time

San Diego Community Living Services (SDCLS) is seeking compassionate and dedicated individuals to join our team as Direct Support Professionals (DSPs). Our teaching program empowers individuals with disabilities to live independently, make choices, and actively participate in their community.

Schedule:

• Weekend Shift (48 Hour shift per weekend)
• Saturday 8:00 AM - Monday 8:00AM (Straight shift)
• (Full-time position with set hours, no flexible scheduling.)

Responsibilities:

• Provide direct support to 1 individual with disabilities in their home and community.
• Assist with daily living activities, skill-building, and community engagement.
• Offer personal hygiene assistance as needed.
• Encourage independence and self-reliance while ensuring a safe and supportive environment.

Qualifications:

• Passion for helping others and making a difference in the community.
• Previous DSP or caregiving experience preferred but not required—training provided!
• Ability to assist with personal hygiene and other support needs.
• Reliable, patient, and team-oriented.
