Care Navigator

The Crisis Care Navigator is a member of the Crisis Services Team who supports individuals experiencing behavioral health crises in outpatient clinics and hospital emergency departments. This role provides rapid engagement, care coordination, and connection to appropriate behavioral health services to ensure timely intervention and continuity of care.
