At Second Chance Case Management, you'll carry a meaningful full-time caseload across Metro Atlanta, conducting nursing assessments that directly determine whether elderly and disabled Georgians receive the Medicaid waiver services keeping them in their homes. No hospital shifts. No overnight shifts. No waiting on a charge nurse to tell you what to do next.

WHY THIS ROLE IS DIFFERENT

This is a salaried, W-2 full-time position at $75,000/year — not per-visit, not contract, not "it depends." You know what you're making. You know what you're doing. You show up, serve your clients, and go home.

• Structured Schedule, Real Flexibility – Full-time doesn't mean 12-hour hospital shifts. You'll manage your Metro Atlanta territory with a predictable schedule built around client visit windows.
• Your Territory, Your Clients -- You'll work Metro Atlanta consistently, building real relationships with clients and their families over time.
• Autonomy With Backup – Run your caseload independently. A supervising RN and dedicated training nurse are a call away when clinical questions come up

WHAT YOUR DAY ACTUALLY LOOKS LIKE

This position supports clients enrolled in Georgia's EDWP, SOURCE, and CCSP Medicaid Waiver programs — some of the state's most impactful long-term care programs.

• Conduct face-to-face nursing assessments in client homes, hospitals, and care facilities across Metro Atlanta
• Analyze client health data and ensure documentation meets Georgia Medicaid program standards
• Partner with RNs, physicians, and case managers on care plan development and reviews
• Maintain timely communication with your supervisor and care coordination team
• Participate in team meetings, required trainings, and ongoing clinical development
• Manage your visit schedule and territory with professionalism and ownership

QUALIFICATIONS

• Active Georgia LPN license in good standing (required)
• Experience in geriatric care, community health, long-term care, or chronic disease management
• Self-directed and accountable — you manage your schedule and caseload without constant oversight
• Strong communicator with elderly clients, families, and care teams
• Reliable, professional, and genuinely invested in the people you serve
• Valid driver's license and reliable transportation required
