Responsibilities
- Ensure continuous care for assigned members across both acute and post-acute environments.
- Conduct thorough biopsychosocial assessments to support care planning and identify the unique needs of each member.
- Work collaboratively with physicians, case managers, and care teams to optimize care coordination to achieve care goals.
- Act as a bridge between members, their families, and healthcare providers to facilitate clear communication.
- Address behavioral and mental health needs of members, linking them to suitable care and resources during crises.
- Provide telephonic psychosocial support while assisting members in understanding and managing their health conditions.
- Connect members with community resources, aiding in benefits and insurance navigation to ensure access to vital services.
- Triage referrals effectively, ensuring members are directed towards appropriate care and resources.
Qualifications
- BSW required; MSW preferred.
- Must hold a valid and current state license.
- A minimum of 3 years in social work, with at least 1 year focused on mental/behavioral health conditions.
- Preferred certifications include Qualified Mental Health Professional (QMHP), Mental Health Technician Certification (MHTC), or Certified Clinical Mental Health Counselor (CCMHC).
- Proficient in assessing and addressing the specific needs of a targeted population.
- Experience with Post-Traumatic Stress Disorder (PTSD) is a plus.
- Strong interpersonal, communication, and problem-solving skills.
- Ability to maintain composure and provide support during crisis situations.
- Meticulous attention to detail and a strong commitment to completing tasks.
- Technologically proficient with Microsoft Windows and general computer applications.
- Highly organized and efficient in working autonomously while collaborating within a team.
- Reliable high-speed cable or fiber optic internet service is required.
- Exceptional communication skills across various platforms, providing timely follow-up on all inquiries.
- U.S. Citizenship is required.
- Successful candidates must pass a preliminary credit and background check before employment.
