Overview

Trans Perfect Health is looking for a full‑time Physical Therapist Assistant (PTA) to join our team in Roosevelt, Utah. The role involves delivering patient‑centered care to residents in senior living and skilled nursing settings.

Key Responsibilities

• Responsible for the treatment of residents with noted disorders or deficits within Physical Therapy scope of practice upon receiving physician order and completion of evaluation.
• Optimize the functionality and skills of patients.
• Completes all necessary documentation via Net Health.
• Collaborate with supervising PT developing programs appropriate to the needs of facility residents while fulfilling PTA/PT co‑signature and supervision requirements.
• Attend all required meetings including care planning, staff meetings, family meetings, etc.
• Assist the facility in meeting the standards necessary to obtain state and federal certification while following OBRA Resident Right Guidelines.
• Follow facility infection control policies and procedures.
• Complete all other tasks that are deemed appropriate for the role and assigned by the manager/supervisor.

Qualifications

• Current or eligible Physical Therapist Assistant license in the state of practice.
