Your Qualifications

• Licensure: Must be a current and unencumbered LPN.
• Experience: Over a year in Certified Home Health as an LPN.
• Skills: Competence in wound care and wound vac.
• Transportation: Reliable means of transport, a valid driver's license, and current car insurance.
• Certifications: Valid CPR certification; additional home health or geriatric care credentials are advantageous.

Responsibilities

• Deliver exceptional care independently to patients in their homes, including conducting thorough assessments and developing tailored care plans.
• Ensure timely and complete documentation of care through HomeCare HomeBase EMR.
