Contract | Days | Full-Time | Onsite | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Typically one full weekend per four weeks required

Key Responsibilities

• Provide direct social work services to patients within an acute care facility setting
• Perform discharge planning and coordinate post-acute care transitions for patients
• Manage a caseload with a ratio of approximately 1:25 during day shifts
• Collaborate with interdisciplinary care teams to assess and address patient social and psychosocial needs
• Commit to working 3 of 5 designated holidays as required by the facility

Required Qualifications

• Minimum of 2 years of acute medical Social Work experience, encompassing both social work and discharge planning responsibilities
• Active California state Social Work license required upon start date (pending license accepted at time of submission)
• Ability to begin assignment as soon as possible
• Completion of a skills checklist prior to submission review
• Two professional references covering at least 1 year of experience within the last 3 years

Preferred Experience / Skills

• Prior travel Social Work experience preferred; first-time travelers will be considered with appropriate screening
• Familiarity with acute care facility workflows, interdisciplinary team collaboration, and patient care coordination
