RN MDS Coordinator (Full-Time & Part-Time)
Linden Place Center for Nursing and Rehabilitation is seeking an experienced and detail-oriented Registered Nurse (RN) MDS Coordinator to join our clinical leadership team. As an RN MDS Coordinator, you will play a vital role in ensuring the timely completion of resident assessments, care planning, reimbursement accuracy, and compliance with federal and state regulations. Key Responsibilities
•
Coordinate and manage the completion of the Resident Assessment Instrument (RAI) process, including MDS assessments, Care Area Assessments (CAAs), and individualized care plans
•
Collaborate with the interdisciplinary team to ensure timely, accurate, and comprehensive resident assessments
•
Maintain and communicate the MDS assessment schedule for all payer sources
•
Participate in daily PPS meetings, weekly Medicare reviews, and month-end reimbursement and compliance activities
•
Gather and validate assessment data through resident interviews, clinical observations, chart reviews, and collaboration with department leaders
•
Ensure all interdisciplinary team documentation is completed accurately and within required regulatory timeframes
•
Monitor clinical documentation to support quality measures, reimbursement, and regulatory compliance
•
Assist with medical record audits, quality improvement initiatives, and staff education related to MDS documentation and care planning
•
Stay current with CMS regulations, PDPM guidelines, and reimbursement requirements
Qualifications
•
Current Registered Nurse (RN) license in the state of North Carolina
•
Minimum of three (3) years of clinical nursing experience, preferably in skilled nursing or long-term care
•
Previous MDS Coordinator experience strongly preferred
•
Current CPR certification
•
Thorough knowledge of MDS/RAI processes, Medicare regulations, PDPM, and care planning
•
Strong analytical, organizational, and time management skills
•
Excellent communication and collaboration skills with residents, families, physicians, and interdisciplinary team members
•
Commitment to accuracy, compliance, and delivering exceptional resident-centered care
