RN MDS Coordinator (Full-Time & Part-Time)

Linden Place Center for Nursing and Rehabilitation is seeking an experienced and detail-oriented Registered Nurse (RN) MDS Coordinator to join our clinical leadership team. We are looking for a compassionate professional who is committed to delivering quality resident care through accurate assessments, interdisciplinary collaboration, and regulatory compliance.

As an RN MDS Coordinator, you will play a vital role in ensuring the timely completion of resident assessments, care planning, reimbursement accuracy, and compliance with federal and state regulations. Key Responsibilities

Coordinate and manage the completion of the Resident Assessment Instrument (RAI) process, including MDS assessments, Care Area Assessments (CAAs), and individualized care plans

Collaborate with the interdisciplinary team to ensure timely, accurate, and comprehensive resident assessments

Maintain and communicate the MDS assessment schedule for all payer sources

Participate in daily PPS meetings, weekly Medicare reviews, and month-end reimbursement and compliance activities

Gather and validate assessment data through resident interviews, clinical observations, chart reviews, and collaboration with department leaders

Ensure all interdisciplinary team documentation is completed accurately and within required regulatory timeframes

Monitor clinical documentation to support quality measures, reimbursement, and regulatory compliance

Assist with medical record audits, quality improvement initiatives, and staff education related to MDS documentation and care planning

Stay current with CMS regulations, PDPM guidelines, and reimbursement requirements

Qualifications

Current Registered Nurse (RN) license in the state of North Carolina

Minimum of three (3) years of clinical nursing experience, preferably in skilled nursing or long-term care

Previous MDS Coordinator experience strongly preferred

Current CPR certification

Thorough knowledge of MDS/RAI processes, Medicare regulations, PDPM, and care planning

Strong analytical, organizational, and time management skills

Excellent communication and collaboration skills with residents, families, physicians, and interdisciplinary team members
