As a Licensed Practical Nurse in Home Health, you will play a crucial role in delivering quality patient care and performing skilled nursing tasks in the comfort of patients' homes.
Key Responsibilities:
- Deliver high-quality clinical services within your scope of practice, adhering to state nursing laws and the established plan of care.
- Document patient visits thoroughly and accurately according to regulatory requirements, utilizing the agency's electronic medical record system.
- Participate actively in care coordination by reporting changes in patients' conditions and attending case conferences.
- Provide care in diverse home situations, respecting clients' backgrounds and circumstances.

Required Qualifications:
- Valid and unrestricted LPN license.
- Current driver's license, vehicle insurance, and access to reliable transportation.
Preferred Qualifications:
- Current CPR certification or willingness to obtain within 90 days of hire.
- Experience in home healthcare.
- Ability to manage multiple tasks effectively.
- Self-motivated and capable of working independently.
- Strong communication, writing, and organizational skills.
- Willingness to work flexible hours to meet client needs.
Compensation details: Annual total cash target pay ranges from $46,564 to $69,846, based on full-time employment. Hourly rates vary between $26.86 and $40.30. Pay is based on productivity and includes per-visit compensation. We also offer a comprehensive benefits package, equity stock purchase options, and 401k contributions, subject to eligibility requirements.
