Community Health Worker- School Based

This position is responsible for assisting patients and other individuals by assessing the needs and resources available within our communities; providing social support and informal guidance regarding navigation and access to community resources; conducting home and environmental assessments to evaluate and address potential barriers to care in support of access to care at our health centers and care plan implementation; participates as an active member of the care team; functions as a support resource for patients in addressing social determinants of health and assists with follow up of patient goals; provides Outreach and Enrollment services assisting patients and families to obtain, maintain and sustain enrollment in their insurance plan of choice.
