Social Worker- PRN

• Psychosocial Assessments: Evaluate patients' and families' emotional, social, and spiritual needs to develop individualized care plans.
• Emotional Support & Counseling: Provide counseling to help patients and families cope with terminal diagnoses, grief, and bereavement.
• Resource Coordination: Assist in accessing community resources, including financial aid, home care services, and support groups.
• Advance Care Planning: Guide discussions on advance directives, DNR orders, and end-of-life preferences to ensure patients' wishes are honored.
• Interdisciplinary Collaboration: Work with healthcare teams to coordinate care, ensuring alignment with patients' goals and values.
• Documentation: Maintain accurate records of assessments, interventions, and care plans in compliance with regulatory standards.
