CarePlus TLC is seeking a dedicated, compassionate, LPN/CNA/Home Health Aid with a passion for serving others to join our team!

• Client is located in Kentwood. 12 hour shifts 7a-7p or 9a-9p.

As a Home Health Aide/CNA at Care Plus TLC, you will play a vital role in assisting patients with daily living activities and providing essential health care support. This position is crucial in enhancing the quality of life for our clients while promoting their independence.

*MUST HAVE VALID DRIVERS LICENSE

*MUST OWN YOUR OWN VEHICLE

Responsibilities:

• Administer prescribed medications including injections, enemas/suppositories, etc as ordered by Physician
• Trach and vent care
• Feeding tube care
• Wound care
• Catheter care
• Provide Activities of Daily Living care as outlined in the Care Plan-bathing, dressing, feeding, grooming, etc.
• Monitor patient’s condition and report changes/concerns to appropriate medical professionals and family members
• Transfers patient with equipment using appropriate safety measures with hoyer lift
• Perform light housekeeping duties to maintain a safe and clean-living space for clients.
• Provide companionship and emotional support to enhance patient well-being.
• Take and record Vital signs including fluid intake and output
• Ability to turn and reposition patients
• Understands First Aid and Emergency Care protocols
• Complete electronic documentation while on shift
• Adhere to HIPAA regulations and maintain patient confidentiality.
• Help with mobility and transportation needs for appointments or errands.

Skills:

• 1 year experience as a licensed nurse is preferred.
• Trach and vent experience is a MUST
• Adequate knowledge of medical terminology
• Successful completion of a LPN or RN license program is a must
• Understanding of Nursing best practices
• Familiarity with Home Health Care, safety and sanitation standards and procedures
• Understanding of Confidentiality and Privacy Laws
• Excellent communication and interpersonal skills… able to communicate effectively with patients and their families.
• Detailed oriented
• Ability to turn and reposition patients
• Ability to undertake a variety of diverse care tasks and work as a team member
• Compassionate and able to handle tough situations as needed
