Job Summary

Assists in the assessment, referral, coordination, and delivery of services for patients. Responsible for assessing and monitoring patients, providing education and support to both patients and their caregivers, developing state‑wide referral resources relevant to the anticipated target population, and collaborating with other members of the team.

Responsibilities

• Clinical case management
• Track and oversee high‑risk cases
• Collaborate with referral coordinator
• Function as member of the triage team
• Other duties and responsibilities as assigned

Qualifications

• Minimum qualifications:

master’s degree in social work, counseling, or closely related discipline.

• Master of Science, Master of Social Work, Masters of Counseling and 2–3 years of experience or an equivalent combination of education and experience required.
• Applicants with an M.A., M.S., M.S.W., or related degree, with Cognitive Therapy/CBT training and experience working with pediatric populations are encouraged.
• Experience delivering exposure therapy to youth with anxiety or OCD is a plus but not required.
• Role is contingent on continued funding.
