Role Description The Licensed Social Worker is a full-time, on-site role located in Garfield Heights, OH, supporting residents and families across Independent Living, Assisted Living, Skilled Nursing, Therapies, and Memory Care. Day-to-day responsibilities include conducting psychosocial assessments, developing care plans, and coordinating services to address emotional, social, and practical needs. The role involves providing counseling and support, facilitating care conferences, advocating for residents’ rights and preferences, and assisting with transitions between levels of care or external placements. The Licensed Social Worker collaborates closely with nursing, therapy, activities, and administrative teams to promote continuity of care, safety, and quality of life. The position also includes maintaining accurate documentation, complying with regulatory standards, and participating in quality improvement initiatives.
Qualifications
- Current state licensure as a Social Worker (LSW or equivalent) and compliance with all professional and ethical standards.
- Experience in long-term care, assisted living, memory care, or related healthcare settings, preferably with older adults.
- Strong assessment, care planning, and case management skills, including the ability to coordinate services and resources for residents and families.
- Effective communication and interpersonal skills, with the ability to build trust, mediate concerns, and collaborate with interdisciplinary teams.
- Knowledge of psychosocial needs of older adults, community resources, and regulations related to resident rights, discharge planning, and continuity of care.
- Proficient documentation and organizational skills, including accurate record-keeping and time management in a fast-paced environment.
- Demonstrated commitment to compassion, cultural sensitivity, and person-centered care aligned with the community’s mission and values.
- Bachelor’s degree in Social Work or related field required; advanced degree in Social Work or related discipline is a plus.
- Ability to work on-site in Garfield Heights, OH, and to adapt to changing resident needs, regulatory requirements, and organizational priorities.
