We are a Medicare-certified home health agency seeking a compassionate and reliable Home Health Aide (HHA) to provide intermittent, field-based patient care in the community. This position is not for continuous, shift-based care, but rather for scheduled home visits to multiple patients throughout the day.

The Home Health Aide will travel between patient homes to provide hands-on assistance with Activities of Daily Living (ADLs) and personal care support, then transition to the next assigned visit.

Key Responsibilities

  • Provide personal care assistance during scheduled home visits, including bathing, grooming, dressing, and hygiene support
  • Assist patients with Activities of Daily Living (ADLs) as outlined in the care plan
  • Provide basic mobility support, including transfers and positioning
  • Observe and report changes in patient condition to the supervising nurse
  • Maintain patient dignity, privacy, and safety at all times
  • Follow established care plans and agency policies
  • Accurately document services provided per agency requirements
  • Travel between patient homes in assigned geographic area

Position Structure

  • Field-based visit model (no continuous or live-in care)
  • Multiple patient visits per day depending on schedule
  • Flexible scheduling options may be available (PRN / Part-time / Full-time depending on census needs)

Qualifications

  • Active Home Health Aide (HHA) certification (as required by Florida regulations)
  • Experience in home health or caregiving preferred
  • Current CPR certification preferred
  • Ability to travel independently between patient homes
  • Strong communication and documentation skills
  • Compassionate, dependable, and patient-focused
  • Ability to follow care plans under RN supervision

Preferred Experience

  • Experience in Medicare-certified home health setting
  • Familiarity with ADL assistance in home-based care environment
  • Prior experience working with elderly or medically complex patients
  • Bilingual (English/Spanish) preferred

Physical Requirements

  • Ability to lift, transfer, and assist patients safely
  • Standing, walking, and bending throughout the workday
  • Ability to drive or travel to multiple homes daily
