We are a Medicare-certified home health agency seeking a compassionate and reliable Home Health Aide (HHA) to provide intermittent, field-based patient care in the community. This position is not for continuous, shift-based care, but rather for scheduled home visits to multiple patients throughout the day.
The Home Health Aide will travel between patient homes to provide hands-on assistance with Activities of Daily Living (ADLs) and personal care support, then transition to the next assigned visit.
Key Responsibilities
- Provide personal care assistance during scheduled home visits, including bathing, grooming, dressing, and hygiene support
- Assist patients with Activities of Daily Living (ADLs) as outlined in the care plan
- Provide basic mobility support, including transfers and positioning
- Observe and report changes in patient condition to the supervising nurse
- Maintain patient dignity, privacy, and safety at all times
- Follow established care plans and agency policies
- Accurately document services provided per agency requirements
- Travel between patient homes in assigned geographic area
Position Structure
- Field-based visit model (no continuous or live-in care)
- Multiple patient visits per day depending on schedule
- Flexible scheduling options may be available (PRN / Part-time / Full-time depending on census needs)
Qualifications
- Active Home Health Aide (HHA) certification (as required by Florida regulations)
- Experience in home health or caregiving preferred
- Current CPR certification preferred
- Ability to travel independently between patient homes
- Strong communication and documentation skills
- Compassionate, dependable, and patient-focused
- Ability to follow care plans under RN supervision
Preferred Experience
- Experience in Medicare-certified home health setting
- Familiarity with ADL assistance in home-based care environment
- Prior experience working with elderly or medically complex patients
- Bilingual (English/Spanish) preferred
Physical Requirements
- Ability to lift, transfer, and assist patients safely
- Standing, walking, and bending throughout the workday
- Ability to drive or travel to multiple homes daily
