Shady Grove Medical Center seeks to hire an experienced Social Worker (LCSW) for our Maternal Care Center who will embrace our mission to extend God’s care through the ministry of physical, mental and spiritual healing.
As a Social Worker (LCSW), You Will
- Completes comprehensive psychosocial assessment with focus on patient’s requirements as they transition to the next level of care
- Collaborates with physicians, nurses, and other disciplines involved with care of the patient to foster a coordinated approach to discharge planning
- Identifies and navigates patient testing and treatment to reduce barriers to patient discharge and preventing delays in patient care; communicates barriers to leadership for resolution and trending
- Communicates with Utilization Review staff on any denials, issues or barriers to discharge
- Identifies services and resources available in the community and assists with patient connection to these services
- Participates in Interdisciplinary Rounds and other patient care conferences
- Documents assessments and interventions according to departmental standards
- Participates in process improvement activities
Qualifications Include
- Bachelor of Social Work (BSW) or Master of Social Work (MSW)
- Minimum of 3 years of experience in diverse clinical setting is required
- Prior case management or discharge planning experience is preferred
- Prior experience with Cerner EMR is preferred
- LBSW, LMSW, LCSW
- Active American Heart Association Basic Life Support (BLS) certification required
Work Schedule
- Employment Type: Variable Part-Time
- Hours per Week/Month: M-F (24hrs/month). Three 8/hr shifts per month OR Six 4/hr shifts per month
- Shift Type: Days
- Weekend Requirements: None
- Holiday Requirements: Follow company holiday calendar
- Shift Differential Eligible: Yes
Pay Range
$34.58 - $51.87
If the salary range is not displayed or if the position is Per Diem (with a fixed rate), salary discussions will take place during the screening process.
